West Scholarship Renewal Application Form PDF

You will complete the application form on your computer using the live fields within the form. To ensure your information is successfully saved in the application fields, please use only Adobe Reader to complete the form. You will be able to save your application in progress on your computer (or extrernal zip drive if desired) and return to it at a later time if necessary. Once completed, you will print the application form in order to mail it.

Alternate ways to aquire the Renewal Application PDF: Should the downloadable renewal application form (above) not be accessable to applicants,it can be acquired through one of the following methods: (Application forms are available between January 1st and April 1st annually.)

• Phone the West Scholarship number at 541.883.3857 or U. S. Bank at 1.800.522.9100, ext. 4882
• Contact the principal or counselor of any high school in Klamath County.

Step 2 of 3: Obtain and prepare your college transcript and optional supplementary documents (from page 3 of the Renewal Application Form PDF)

1. You must obtain your OFFICIAL college transcript, including your most recently completed term, prior to April 1st. Attach the transcript to your application.
2. You may attach any further records, citations, honors, or other documents which you feel may augment your application.

Step 3 of 3: Mail your completed PDF Renewal Application, additional sheets for extended answers, required transcript and optional supplementary documents

When and Where to Mail Your Renewal Application:

• Completed applications must be postmarked by April 1st. Should April 1st fall on a weekend or holiday, the application must be postmarked no later than the first business day following.
